Tire Rack will mail a claim form (also available here) and a copy of your invoice to your billing address within 1-2 weeks. You will need to complete this form and submit it, along with your invoice (website order placed page and order/shipment confirmation emails not accepted). An invoice will also be shipped with your order.
If you do not receive your claim form by mail within two weeks of shipment, please download a copy of the form here, or contact Tire Rack's customer service department by calling 888-981-3953.
