To receive a $60 Firestone Prepaid Mastercard, you must submit (i) an official claim form online and (ii) an original or legible copy of the invoice for the qualifying Firestone tires (website order placed page and order/shipment confirmation emails not accepted for this portion of the offer). An invoice will also be shipped with your order.

Tire Rack will mail a copy of your invoice to your billing address within one to two weeks. An invoice will also be shipped with your order. If you have not received your redemption form by mail within two weeks of shipment, you can download a copy of the form from the website or contact Tire Rack's customer service department by calling 888-981-3953.
